STATEMENT OF WAIVER OF LIABILITY AND ASSUMPTION OF RISK RELATED TO CORONAVIRUS/COVID-19

The World Health Organization declared the coronavirus, COVID-19, a pandemic on March 11, 2020. COVID-19 is defined as a highly contagious virus and is known to spread by person-to-person contact and is an airborne contagion. Since that time federal, state, and local governments and federal/state agencies have recommended the following protocols to promote safety: social distancing, wearing masks, frequent hand washing and use of hand sanitizer, as well as, limiting congregations to smaller groups of people.

Henceforth, you acknowledge the contagious nature of COVID-19 and thus voluntarily assume all risks of attendance and participation at the Connection Hill Primitive Baptist Church, Inc., and that you and your family may be exposed to or infected by COVID-19, at the Connection Hill Primitive Baptist Church, Inc. facility, either in the Worship Services, parking lot, Church School, Bible Study or any other faith based activities.

You further acknowledge that the risk of being exposed to or infected by COVID-19 may result from actions, omissions, or negligence of yourself or others, including but not limited to Connection Hill Primitive Baptist Church, Inc. employees, staff, contractors, volunteers, members, attendees, participants, and families. You agree on behalf of your heirs, successors, assigns and next of kin that in assuming all foregoing risks, to release, waive liability claims against, indemnify, hold harmless, defend and covenant NOT TO SUE the Connection Hill Primitive Baptist Church, Inc., the Pastor, Deacons, Trustees and each of its respective ministries, departments, directors, administrators, teachers, officers, agents, representatives, volunteers and employees from any and all actions, claims, demands or liabilities, including without limitations, those for personal injuries, disability, death or property damage, that you may suffer. You further agree to accept sole responsibility for any illness, injury, disability, or death to you or your family, including all claims that may arise resulting from any of these. You further agree to abide by the policies and procedures established by Connection Hill Primitive Baptist Church, Inc. to protect attendees and staff, including but not limited to wearing masks, and social distancing as required while in attendance at Connection Hill Primitive Baptist Church
