Medical events do not occur in isolation. They unfold within complex systems shaped by context, conditions, constraints, and human decision-making. This workshop is designed to move beyond outcome-based reviews and individual blame, focusing instead on how work actually happens and how meaningful learning can be extracted from real-world medical cases.
Participants will explore:
How Human & Organizational Performance (HOP) applies to medical and healthcare environments
Why traditional case reviews often fail to produce lasting learning
How facilitated learning and learning team approaches can improve understanding and response
What it means to learn from a medical case without simplifying complexity
This workshop is intended for healthcare leaders, safety professionals, clinicians, and others interested in system-focused learning and improvement.
The workshop will be held at Hotel Santa Fe.

Led by Todd Conklin, with RaDonda Vaught
Day One establishes the shared language and foundational concepts that will be used throughout the workshop, including Day Two, and features RaDonda Vaught speaking to her firsthand experience of the medical case examined during the session.
Facilitated by Todd Conklin, the session focuses on understanding how the event unfolded within its full operational context and how decisions were shaped by the system in which the work occurred.

Common language used in Human & Organizational Performance that will be applied on Day Two
Discussion of the Five Principles of HOP
Examination of a real medical case through a system-focused lens
RaDonda Vaught’s personal account of the medical event and its broader context
Understanding context, pressures, and constraints influencing decisions
Facilitated learning focused on understanding rather than judgment
With continued participation from Todd Conklin and RaDonda Vaught
Day Two builds directly on the shared language, concepts, and case learning established on Day One. Todd Conklin and RaDonda Vaught remain engaged throughout the workshop, supporting facilitated learning, discussion, and reflection as participants move into application and practice.
